Case Study using 3D Printed Metal Palatal Expander by Dr Richard Cousley.
Case Study using 3D Printed Metal Palatal Expander
We are delighted to present this case from Dr Richard Cousley using the 3D Printed Metal Palatal Expander, available exclusively from DB Orthodontics.
This patient is a relatively mature 13 year old girl, with moderate dental crowding and where both arches are constricted. The 3D printed RME was fitted within weeks of the patient’s consultation (when a scan was taken) and following digital design of the appliance by Studio 8 (Fig a). The appliance was made and fitted without the need for additional appointments or separators.
The RME (Fig b) fitted beautifully, and was very retentive before even adding cement. Molar tubes were added to the molar shells, during fabrication, so that fixed appliance treatment could proceed after 3 weeks, while the RME remains in place for up to 9 months for skeletal stabilisation. The photo (Fig c) shows the expansion after 3 weeks activation: twice daily for one week followed by two weeks of daily activation. The CBCT scan (Fig d) shows almost parallel opening of the mid-palatal suture.

Embracing Digital Orthodontics: What are the advantages?
FAST
The use of digital processes cuts down turnaround times, and so the time between taking the intra-oral scan and receiving the custom-made appliance is reduced.
EFFICIENT
Innovative techniques used when designing and making appliances result in fewer appointments being needed and less time being spent chairside.
CONVENIENT
Scans are accepted through a variety of channels, this ensures that it is quick and simple for clinicians to send cases in a way which suits them while eliminating the need for alginate/silicone impressions.
PRECISE
Cutting edge, digitally designed and 3D printed appliances provide full customisation and high precision.
COMFORTABLE
Putting patient comfort at the forefront of design, our 3D printed appliances eliminate the need for bands and separators.

Expansion - Achieving Precise Results with Comfort
Discover our full range of 3D printed palatal expanders, including Quad helixes and TPAs. These state-of-the-art appliances eliminate the need for separators and molar bands, reducing the number of appointments and chair time while enhancing patient comfort. Orthodontists benefit from easy-to-fit precision appliances, thanks to the precise nature and full customisation of the 3D design and printing process.
TAD Appliances - Streamlining Expansion
Combine TAD Appliances with Infinitas Mini implants for a precise digital workflow and 3D printed expansion appliance. This allows for a single appointment protocol for both the expander and mini-implants, enabling a quick transition from an intra-oral scan to expansion within a few weeks. This approach even works for MARPE in mature adolescents and young adults, achieving direct skeletal expansion effects.
Autograph aligners – discreet and effective alignment
Autograph aligners combine effective alignment with the sophistication of clear, discreet aligners. Skilled dental technicians use the latest technology to plan the movements and axis of each tooth based on your instructions. This meticulous planning ensures a reliable and predictable clinical outcome.
Retention – maintain your perfect smile
The Maintain system comprises three key elements: a bonded retainer, a thermoformed transfer jig, and an additional thermoformed retainer. These components work in harmony to provide maximum retention and permanent support while being easy and convenient to fit.
Scans can even be taken with brackets in place, as skilled technicians can remove them digitally. This makes the Maintain system ready to fit at the de-bond appointment.
