From Digital Planning to Predictable Expansion: Case Study By Dr Richard Cousley

This case involves a 19 year old male patient who presented with a mild Class III malocclusion, on a mild Class III skeletal relationship, complicated by increased lower anterior vertical proportions. He had bilateral buccal crossbites and a left functional mandibular displacement.

A CBCT showed that his mid-palatal suture was in the late stage of maturation prior to bony fusion, and that the upper molars were buccally inclined (along with clinically visible lingual tipping of the lower buccal teeth).

The 3D digital planning process, by DB Orthodontics DB Digital Laboratory, involved merging of the CBCT with a digital model to create a fully customised hybrid RME appliance utilising bone anchorage in the anterior palate. This expander was then fitted at the patient's second-ever clinical appointment and the first expansion turn was performed by him on the same day. This single-visit approach was only feasible with the use of a matching mini-implant guidance stent created by the same digital planning process.

The patient underwent three weeks of expansion, at the rate of two turns (0.4 mm) per day. A midline diastema opened by day six, and reached 6.5 mm at the crestal level by day 21. There was almost parallel palatal suture opening of up to 5 mm radiographically. The expander was then activated by a further 1mm.

Dr Cousley's case highlights how skeletal anchorage and accurate digital planning can deliver efficient and predictable expansion, even in patients approaching skeletal maturity.
